Appointment Request

Prior to entering our office for an appointment, please call, email or text our office to get in touch with us personally, so we can confirm an appointment for you.  The appointment request feature is only a request to accommodate your preferred day and time.  We will do our best, but please note, your appointment is not booked until you get a confirmation from us!
